An Advance Care Directive (ACD) empowers you to:

make clear legal arrangements for your future health care, end of life, preferred living plans and other personal matters
appoint Substitute Decision-Makers to make informed decisions on your behalf if you are unable to make them yourself

Come along to a free information session where you gain resources and understanding on how to complete an ACD.
